Eva Green’s Sybilla loses her son in the theatrical cut with little echo. In the extended cut, after her son dies (poisoned by his own mother to prevent him becoming a monster like Guy), she shaves her head and descends into a silent breakdown. When she later asks Balian to marry her, it is not romance; it is a desperate gasp for a single thread of goodness.
In the theatrical cut, Balian (Orlando Bloom) is a vaguely sad blacksmith. In the extended cut, we learn he is a guilt-ridden sinner: his wife (whose soul the local priest claims is in hell) killed herself, and Balian murders that same priest after being taunted. This crime is why he seeks penance in Jerusalem. Without this, his journey from despair to grace has no foundation.
